The following documents must be received prior to/or at time of Orientation

  • Copy of Driver License
  • Proof of Car Insurance
  • Social Security Card
  • Physical Exam (within 6 months)
  • TB test or Chest X-Ray
  • Current CPR Card
  • Green Card or Work Permit
  • 75hr HHA Certificate or CNA Certificate
  • 4hrs Aids and OSHA Original Certificate & any updates
  • Domestic Violence Certificate
  • Alzheimer’s Certificate
  • Must have updated Level 2 background, including fingerprints
