Risk Assessment in the Home
Elderly patients are especially vulnerable to fall risks, unintentionally skpping meals, medication, or have lots of questions related to the multiple medications prescribed by more than one provider . The majority routinely take more than five prescriptions and occasionally unintentionally deviate from their prescribed regime. Our Registered Nurse would be able to work with your physician’s office to ensure that your loved one questions are answered by the appropriate party, offer some guidance, and consider appropriate steps during the home assessment process.
RN Home Assessment
Atlantic AllCare offer complimentary assessment as part of our intake process. An experienced Registered Nurse will be assigned to you or your loved one. They will assess your home care needs and communicate with your physician’s office about your care as appropriate.
They will then communicate and coordinate these needs effectively to the right sources.
Home Modifications
Atlantic AllCare Case Management can consult with the client on the potential home modifications that may be needed and make recommendations based on the needs developed from our initial evaluation. As these needs change, Atlantic AllCare can make continuing recommendations to ensure that such needs continue to be met at the highest standards possible, whether it’s installing grab bars in the bathroom, removing rugs, or adding a ramp.
